As for the elimination of the hormone therapy - I have been there done that already.  About three years ago Medco mail order pharmacy stopped carrying my hormone medication.  I went round and round with them.  No generic replacement, company stopped making it.  One lie after another.  I persisted with them and finally got an email from them that they would not cover the med period because the classified it as a Class 1 opiate!  I am still able to get it thru a local pharmacy but have to pay $58 every month.  I used to pay $30 a 90 day supply.  I talked to a pharmacist and he told me as soon as Obamacare was passed their was a memo passed to the drug companies to try to eliminate as many hormone drugs as possible as the Dept of Health and Human Services said the side effects do not warrant keeping this on the market anymore.
